Concussion Requirement Assignment



Please go to this site and complete the concussion requirement test.



Register as a student and print out the completion certificate.



In order to receive your clearance card to participate in sports you must take your

• Physical forms that are completed by a doctor. (Mrs. Morken in the front office has them.
• Concussion completion certificate (if you have taken the concussion test previously at WF and have a copy on file you do not need to take it again).​
• ​5 pages from student handbook must be signed (see previous page for link to 5 pages)
